The U.S. Virgin Islands faces a critical shortage of long-term care facilities. Herbert Grigg Home for the Aged on St. Croix is the territory’s only government-operated nursing facility, with 26 beds. Sea View Nursing Home on St. Thomas closed in 2020 after 17 years of operation. Residents who need skilled nursing care beyond what is available locally are sometimes placed at facilities outside the territory at a cost of $72,000 to $108,000 per year.
The USVI legislature has advanced a $20 million plan to rebuild nursing home capacity, but new facilities are not yet operational.
The territory does not participate in the Long-Term Care Partnership Program.
Medicaid
The U.S. Virgin Islands Medicaid program operates with a capped federal allotment. Long-term care services available through Medicaid are significantly more limited compared to state programs. The shortage of facilities means that even when coverage exists, finding available care is a challenge.
